What’s a typical day as a Case Manager - Service Coordinator? You’ll be:

• Providing service coordination to individuals residing in the community

• Maintaining current knowledge of Department of Health (DOH) policies, procedures, and regulations related to service coordination

• Assisting individuals and families in accessing programs, benefits, and other opportunities to improve their quality of life

This job might be an outstanding fit if you:

• Have a valid NYS Driver's License and reliable transportation

• Have a Bachelor's degree and at least 2 years of experience providing Service Coordination and information, linkages, and referrals for individuals with disabilities; OR

• Have an Associate's degree and at least 3 years of experience providing Service Coordination and information, linkages, and referrals for individuals with disabilities; OR

• Have a High School Diploma/GED and at least 4 years of experience providing Service Coordination and information, linkages, and referrals for individuals with disabilities; OR

• Have successfully served as a Regional Resource Development Specialist for 1 year in the HCBS/TBI Waiver Program
