Reliance Home Health Caregivers Scheduling and staffing assistants ( Compassionate Living Home Inc) in Bolingbrook, Illinois

Overview:

The main focus of this position is to coordinate the development and implementation of supported living services with the consumer and employee and ensuring the delivery of quality services. This coordination occurs through the direct supervision of employees, supports for consumers, and ongoing communication with family, community, funding and licensure sources. All coordination must be conducted within contract, state and federal regulations, as well as Company Policies and Procedures. This position reports directly to the Program Coordinator.

Responsibilities:

· Person Centered Planning functions and Individual Support Planning Functions:

o Actively participates in the person-centered planning process, including development and implementation of Person-Centered Plans (PCPs) and the Individual Support Plan (ISPs);

o Design and implement support and teaching strategies targeted at assisting consumers with reaching their outcomes as outlined in the consumer’s ISP.

o Provide supervision and training to Direct Support Professionals regarding the implementation of the PCP, ISP and teaching strategies.

o Evaluate and revise support and teaching strategies as necessary to ensure progress toward achievement of outcomes and action steps.

o Regularly monitor the implementation of the PCP and ISP.

o Ensure that services and supports are provided in the PCP and ISP.

o Complete weekly outcome report and provide to the Program Coordinator.

o Facilitate opportunities for natural supports.

· Consumer Record Related functions:

o Maintenance of consumer’s file including, but not limited to:

§ Consumer profile data with emergency contact numbers;

§ Copies of Medicare/Medicaid cards for all supported living sites, current Release of Information forms, Consent forms, and current signed Consumer Rights statement;

§ Copies of guardianship paperwork if applicable;

§ Financial records including documentation of all account spending, personal spending, and household spending;

§ Daily progress notes, monthly reviews, and other consumer related documentation;

§ Medical information including documentation of necessary medical appointments, lab results, immunizations, physicians medication orders, Medication Administration Records, and controlled drug count sheets;

§ Monthly Health Summary;

§ Monthly Client Activity Calendar

· Quality Assurance Related functions

o Maintain confidentiality of all consumer’s Protected Health Information used for treatment, payment, health care, etc. as mandated by HIPAA.

o Review all forms, notify Program Coordinator of any incidents, and provide oversight for the follow up plan with staff.

o Maintain communication with family members, guardians, staff and Program Coordinator on a regular basis, particularly regarding policies, issues, and concerns.

o Prepare and submit reports that are required by contract, funding sources, or regulations.

· Supervision/Training Related functions

o Coordinate in-service trainings needed to assist staff in fulfilling responsibilities related to consumer directed support.

o Provide oversight with documentation of the Medication Administration Record with checks of the MAR at least once per week.

o Monitor and record all required trainings for staff and schedule updates as needed to remain current.

o Complete a monthly staff schedule ensuring appropriate levels of staff coverage.

o Provide direct support in providing services needed to protect the health and safety of the consumer, in a staffing crisis.

o Provide training and oversight for staff regarding Medicaid Waiver guidelines and licensing and certification standards.

o Communicate with the Interdisciplinary Team about concerns and findings.

o Coordinate and assist the staff with maintaining positive relationships.

o Be available to respond to staff in case of an emergency.

o Perform On-the-Job Training (OJT) for all support staff.

o Perform on-site evaluations for all field staff.

o Complete all annual evaluations and submit to Program Coordinator one month prior to hire date anniversary to ensure timely reviews.

· Other job functions

o Coordination of consumer benefits including Medicaid, Medicare, SSI, SSA, and Food Stamps.

o Securing community resources for consumers supported.

o Maintain consumer files per licensure and company regulations.

o Ensures compliance with licensure guidelines.

o Scheduling and completion of consumer medical needs.

o Assist the Program Coordinator in teaching the training classes, as requested.

o Maintain and promote professional manner for self, and support staff within all communications.

o Assist the Program Coordinator with other duties, as assigned and with approval.

Minimum Qualifications:

· BA/BS Degree required in social work, psychology, human services, or related field and/ or.

· Three-year experience in the field of developmental disabilities.

· Supervisory experience preferred.

· Personal vehicle which is insured and meets state safety/licensure requirements.

· Valid driver’s license.

· Must successfully pass criminal background and Employment Disqualification List checks prior to working with consumers:

· Must complete Company orientation.

Work Hours:

· Days, evenings, weekends and 24 hour on-call for emergency response.

· 10 hours of administrative time weekly.

Job Type: Full-time

Pay: $17.00 - $19 00 per hour
